First Appointment

Please arrive 15 minutes prior to your scheduled appointment so that we can confirm your information and review your completed New Patient Forms.

Please bring the following information with you to our office:

Follow-up Appointment

Please arrive 10 minutes before your scheduled time and check in at the reception desk upon arrival. You will be seen by appointment and arrival time.

Please remember to bring your insurance card to each visit. Bring a list of your medications and inform the doctor of any changes to your medications since your last visit. Let you doctor know if you have been hospitalized or had any out patient surgery or procedure since your last visit.

Review your medications and determine if you require any new or refill prescriptions.

We strive to keep on schedule and apologize when our appointments to see the doctor fall behind due to emergencies and other unforeseen delays. We do our best to accommodate ALL of our patients, and your understanding is greatly appreciated.

Please remember that we collect all co pays, deductibles or uninsured amounts that the time of your visit. We accept Mastercard, VISA, personnal checks and cash.

Cancellations

Please notify us as soon as possible if you are unable to keep an appointment or need one changed so that we can accommodate another patient. Failure to cancel 24-hours prior to the appointment time may incur a fee charged directly to you.
